RE: Collection Due Process—Levy
Dear Ms. Logan
I received your letter dated August 25 from the Appeals office, acknowledgment of my case for consideration in your Bailey’s Crossroads Appeals Office. I request a change of venue for my appeals hearing. I do not live in VA. I am 85 years old and my wife is 87 and in painful health. My wife is suffering from constant pain of Fibromayalgia and Spinal Stenosis resulting in severe pain in arms and legs requiring pain pill every four hours, which augments her Alzheimer’s disease. My physical condition is limited because of my age and cracked ribbed injury that is gradually worsening and terminated my ability to work. I also have extensive dental bills and have been forced to terminate my coverage of a Medicare supplemental plan because I can no longer afford it. I’m too old to travel to VA please reschedule hearing in Greenville or Columbia. Also please send power of attorney forms and instructions so I may have a friend help me. I am too old to manage my affairs.
Sincerely,
Henry Patriot
