Dear Ms. Hurst,

You sent me your letter dated April 4, 2007, copy attached. I want to file all tax returns due and to obey the law. However, all of my records were destroyed in a house fire and I do not know what to do.

My CPA and I have written the IRS many times to gather the information we need to comply with the law. However your agency has not helped us one bit.

You state that I received for unknown years, “a fully taxable pension distribution of $12,948”. What years does this apply to?

Mainly, what is this? Can you tell me who sent what when? I have no recollection of receiving this. I didn’t have taxable savings bonds distributions of the amount you listed.

For the years in question, I have not had my notice of CDPH. I am entitled to a face to face CDPH.

I am 88 years old and in poor health and suffer from diabetes, arthritis, memory loss, poor vision, shortness of breath and other related old age illness.

Can you or someone help me?
