Please provide a summary of all invoices showing the dates of service, amounts paid and if possible the names of the Doctors.

Please return the completed Certificate of Authenticity, the summarized printout and copies of invoices to me at the above address no later than September 30, 2009.

This is in reference to a legal matter with the IRS and you have the option of providing the above information to me now, or you may be subpoenaed as a material witness in a court proceeding at a later date in time.
