Subject: Collection Due Process Hearing

Dear Ms Kenisky:

I received your letter indicating that you scheduled an equivalent hearing. This is not the hearing I requested and will not attend. As mentioned in my last letter, when you sent me the letter giving me the opportunity for a CDP hearing, there was no CDPH form enclosed. The 30 day period you mentioned of started when I received the CDPH form. You received my request to a CDP hearing shortly after. I am still requesting the Collection Due Process hearing. I would like to reiterate that it is still my intention to continue with my CDP hearing and have no intention of withdrawing my request. Feel free to contact me at your earliest convenience.
